Our Informatix Health team provides warm compassionate Homemaker & Home Health
Aide services to our veterans. We support veterans with daily living tasks, enabling independence, dignity, and confidence. Our assistance promotes autonomy and self-worth, helping them live life to the fullest.
In addition, we extend support to veterans facing isolation or to those overwhelmed by caregiving exhaustion
Informatix Health is contracted with the Managed Care Support Contractors that administer the Veterans Health Administration’s Community Care Network (VACCN)
What does in-home support for veterans include?
Personal care, bathing, hygiene, continence, and toileting care.
Offer companionship, engage in conversation, participate in activities that promote mental stimulation and social interaction to combat lonliness and isolation.
Assist using mobility aids such as walkers or wheelchairs.
Medication management, reminder to take prescribed medications, keeping track of medication schedules, opening medication packaging.
Prescription pick-up, running errands.
Light household chores, tidying up, laundry, changing bed linens, maintain a clean and safe living environment.
What do I do to get services at home?
Step 1:
Make an Appointment with your VA Doctor – Ask your doctor to evaluate your need for In-Home Services through the VA Community Care Network.
Step 2:
Prepare your DD214 and fill out your 10-10 EZ Form.
Step 3:
Call Informatix Health at 508-388-2020 and look for Melissa, our VA Program Director.
